Medical Illness
The District of Columbia requires us to have, on file, a medical record of each child. All children must have a current physical. Please return the completed medical form before your child’s first day of school.
For the protection of all children your child should be kept at home (WE CAN NOT ACCEPT HIM/HER AT THE CENTER) if he/she shows any of the following symptoms:
· A temperature
· Diarrhea or vomiting
· A rash
· Nasal discharge that is green/yellow or milky
· Discharge from the eyes or ears
Below is a list of common communicable diseases:
· Strep throat (child needs to be on medication for 24 hours)
· Green/runny nose (needs to turn clear or a note from a Doctor)
· Pinworm (note from the Doctor)
· Viral infections (note from the Doctor)
· Infected ear or glands (note from the Doctor)
· Measles (symptom free plus note from the Doctor)
· Mumps (symptom free plus note from the Doctor)
· Chicken Pox (all pox need to be crusted over)
· Scarlet Fever (symptom free plus note from the Doctor)
· Head lice (nits and lice free – parent must wait while teacher checks)
· 5th disease (note from Doctor)
· Ghardia (note from Doctor)
· TB (Note from Doctor)
· Hepatitis B (Note from Doctor)
If your child has been exposed to any disease and/or illness or shows symptoms please keep him/her home.
The note from the Doctor must be on the Doctor’s letterhead or prescription pad. The note must state what illness your child had, that they are no longer contagious, and that your child may return to school. Your child’s note must have the current date and be signed by the Doctor.
